Responsibilities

The Hospice Aide, under the supervision of a registered nurse, provides personal direct patient care services as outlined in the Plan of Care.

  1. Reports patient condition and environment to the Executive Director or designee after each encounter, following the Care Management process as needed.
  2. Assists with ADLs such as mobility, transfers, walking, grooming, bathing, dressing, eating, and toileting during each visit.
  3. Performs incidental household services essential to the patient's health at home to prevent or delay institutionalization, as needed.
  4. Participates in QAPI activities, identifying issues and reporting to the administrator as needed.
  5. Provides emotional support and compassionate care to patients and families regularly.
Education and Experience

License Requirements:

  1. Successful completion of a competency evaluation prior to patient care.
  2. Current CPR certification.
  3. Valid driver's license, vehicle insurance, and access to reliable transportation.

Additional State-Specific Requirements:

  1. AL: Certification preferred.
Skill Requirements
  1. Mature and capable of handling the demands of a hospice aide role.
  2. Ability to read, write, and follow directions accurately and promptly.
