Job summary

Continuum Hospice is seeking a Full-time Hospice Aide (CNA/HHA) to provide hands-on end-of-life care in patients’ homes, working with a nursing team to meet physical, emotional, and personal needs. Schedule: Sunday-Thursday, 8:00a-4:30p, serving Sonoma & Marin Counties.

Requirements include High School Diploma or GED, valid HHA and CNA licenses, CPR if mandated, and a valid driver’s license with reliable transportation. Competitive benefits and growth opportunities are offered.

Qualifications

High School Diploma or G.E.D. required.
Valid Certified Home Health Aide (HHA) license.
Valid Certified Nursing Assistant (CNA) license required.
CPR certification required if state mandated.
Current driver’s license and reliable transportation.

Responsibilities

Provide personalized care following the care plan for patient comfort and well‑being.
Assist with personal care such as bathing, hygiene, bedding, clothing, and grooming.
Deliver exceptional customer service and ensure patient satisfaction.
Support patients’ families by sharing knowledge and guidance.
Maintain accurate documentation in patient records and agency systems.
