Job summary

St. Croix Hospice is seeking a compassionate Hospice Aide to support patients and families in home settings. You will perform direct personal care, assist with mobility and meals, and help maintain comfort and dignity on the patient’s end-of-life journey.

The role emphasizes accurate documentation, adherence to HIPAA, and coordination with the RN Case Manager to follow each patient’s Plan of Care.

Qualifications

• Must have an active CNA license in the state of practice (Missouri).
• Must be at least 18 years old.
• Ability to pass background study, consumer report and drug test.
• Valid State Driver’s License with dependable vehicle and insurance.

Responsibilities

• Provide personal care including bathing, dressing, oral hygiene, and bed linen changes.
• Assist patients with mobility, transfers, and prescribed exercises.
• Assist with meals, feeding, and light household tasks as assigned.
• Monitor patient condition changes and report to the RN Case Manager.
• Administer routine skin care to prevent breakdown and ensure comfort.
• Support medication reminders per state regulations.
• Maintain a clean, safe, and organized home environment.
• Provide emotional support and respite for family caregivers.
• Use equipment safely and document care activities per HIPAA standards.
• Participate in quality assessment and performance improvement activities.

The Hospice Aide provides direct support and personal care for hospice patients, to assist in their day-to-day needs, provide comfort and enhance their quality of life.
