Job summary

Heart to Heart Hospice in Seattle is seeking a compassionate Hospice Aide (CNA) to provide direct patient care under RN supervision, following the Plan of Care. The role emphasizes safe transfers, patient comfort, and accurate documentation.

Responsibilities include infection control, respectful communication with patients and families, timely charting, and adherence to state and federal regulations. Requires active CNA license, high school diploma or GED, and driving eligibility; EMR experience

Qualifications

• High School Diploma preferred or GED.
• Must have an unencumbered CNA certification.
• Current CNA license required.
• At least one year of Hospice Aide or Nursing Assistant experience in relevant settings.
• EMR experience a plus.

Responsibilities

• Provide direct patient care under RN supervision and Plan of Care.
• Assist patient with safe transfers and ambulation per policy.
• Report and document changes and care provided for continuity of care.
• Practice infection control principles.
• Communicate respectfully with patient, family and staff.
• Submit documentation in a timely manner.
• Perform other duties as required.
• Ensure compliance with laws, Medicare regulations and policies.

Hospice Aide - CNA

What You Must Have:

• High School Diploma preferred or GED
• Must have an unencumbered certification according to the applicable Nurse Aide Registry
• Licensure: Current CNA license
• At least one year experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice or long term care facility
• Demonstrates interest in the welfare of ill and elderly; a heart for providing compassionate care
• High integrity, including maintenance of confidential information
• EMR experience a plus
• Must have current driver's license, reliable transportation, and provide ongoing valid and current auto liability insurance

What You Will Do:

• Provide direct patient care to patient under direction of the RN and according to the Hospice Aide/HomemakerPlan of Care.
• Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy
• Provide necessary skills to appropriately report/communicatechanges and document pertinent information and care provided to patient to ensure continuity of care.
• Practice accepted infection control principles.
• Promote positive, supportive, respectful communication to patient/family and other employees
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
