Job summary

AdventHealth in Florida is seeking a full-time Hospice Aide to provide personal care and support to patients per the care plan, document observations, and assist with activities of daily living while traveling across Volusia and Flagler Counties.

The role requires documentation, compassion, and end-of-life care skills; you will inform the nurse of changes and follow the established schedule.

Qualifications

• High School Grad or Equiv [Required].
• Experience in the health care field [Required].
• Hospice experience [Preferred].
• Hospice Aide training from an approved 75 hour training course [Required].

Responsibilities

• Provides personal care to patients according to the assignment details and care plan. Documents patient care and observations timely and appropriately. Participates in end-of-life care for patients and families.
• Informs the registered nurse of any changes in the patient's condition.
• Follows the schedule of care, including making several visits per day with travel.
• Assists patients with activities of daily living, such as bathing, dressing, and grooming.
• Monitors and records vital signs, including temperature, pulse, and respiration. Supports patients with mobility and exercise as prescribed.
