Job summary

Trinity Health in Monticello is seeking a compassionate Home Care Aide (CNA) to provide paraprofessional services to home care clients, supporting the care plan developed by the primary nurse and prescribed by the physician, in compliance with the Nurse Practice Act and applicable regulations.

The role is full-time and requires delivering personal care, assisting with household tasks, monitoring vital signs, and documenting services with attention to safety and ethics in a mission-driven health

Qualifications

• High School diploma or GED.
• Completion of a federally compliant training program and competency evaluation.
• At least one year of experience as an aide in a healthcare setting.
• Valid driver’s license and reliable transportation.
• Ability to work independently and maintain professional relationships with clients and families.
• Commitment to ethical standards and compliance with agency policies.

Responsibilities

• Deliver personal care services such as bathing, grooming, dressing, and mobility assistance.
• Assist with household tasks including meal preparation, light housekeeping, and laundry.
• Monitor and record vital signs as directed.
• Support clients with self-administered medications (no direct administration).
• Use safety equipment appropriately to ensure client well-being.
• Document services accurately and submit timely reports.
• Communicate changes in client condition to the RN supervisor.
• Participate in case conferences and ongoing training.
