Job summary

Grace Hospice MN is seeking a dedicated Hospice Aide to provide compassionate personal care and support to patients and families under the supervision of the Hospice Aide Manager and RN Case Manager.

Responsibilities include bathing, mobility assistance, vital signs, and care plan documentation, with a focus on dignity and HIPAA compliance. Full-time position in Minnesota with benefits and growth opportunities.

Qualifications

• Nursing assistant certification in Minnesota and active registry status.
• At least one year of experience as a nursing assistant or home health aide preferred.
• Valid Minnesota driver's license and daily access to a vehicle.
• Ability to use electronic charting on tablet or mobile phone.
• Strong communication and interpersonal skills.
• Compassionate approach to hospice care.

Responsibilities

• Assist with bathing, dressing, grooming, oral care, skin care, and incontinence care.
• Assist with ambulation, transfers, repositioning, and mobility.
• Obtain and document vital signs.
• Provide light housekeeping as per care plan.
• Complete documentation accurately and on time.
• Communicate changes in patient condition to the clinical team.
• Maintain confidentiality and HIPAA compliance.
• Treat patients, families, coworkers, and community partners with kindness and respect.

Position Summary

Under the supervision of the Hospice Aide Manager and RN Case Manager, the Hospice Aide provides personal care, comfort, and support to patients and families during an important time in their lives.

Responsibilities

• Assisting with bathing, dressing, grooming, oral care, skin care, and incontinence care
• Assisting with ambulation, transfers, repositioning, and mobility
• Obtaining and documenting vital signs
• Providing light housekeeping and other support outlined in the patient’s care plan
• Completing required documentation accurately and on time
• Communicating changes in a patient’s condition to the appropriate clinical team member
• Maintaining patient confidentiality and following HIPAA requirements
• Treating patients, families, coworkers, and community partners with kindness and respect

Qualifications

• Current Minnesota Nursing Assistant Certification and active status on the Minnesota Nursing Assistant Registry
• At least one year of experience as a nursing assistant or home health aide preferred
