Responsibilities

The Hospice Aide, under the supervision of a registered nurse, provides direct personal care services to patients as outlined in the Plan of Care.

  • Reports on patient condition or environment to the Executive Director or designee, following the Care Management process as needed.
  • Assists with ADLs such as mobility, transfers, walking, grooming, bathing, dressing, undressing, eating, and toileting during each patient visit.
  • Performs incidental household services essential to the patient's health at home, helping to prevent or delay institutionalization.
  • Participates in the QAPI (Quality Assessment & Performance Improvement) process by identifying issues and reporting to the administrator as needed.
  • Must have reliable transportation.
Education and Experience
License Requirements
  • Successful completion of a competency evaluation prior to patient care.
  • Current CPR certification.
  • Valid driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.
Skill Requirements
  • Mature and capable of handling the demands of a hospice aide position.
  • Ability to read, write, and follow directions accurately and promptly.
