Job summary

Community Hospice & Palliative Care in Jacksonville, FL seeks a compassionate Hospice Aide to provide hands-on personal care to terminally ill patients, following the plan of care, with weekend and holiday flexibility. Shift: 7 a.m.–7 p.m., 36 hours weekly at the Primary Center: One Shircliff Way.

Applicants must hold a Florida CNA license, CPR/BLS, and a valid driver’s license; strong communication and documentation skills are essential. A $1,000 sign-on bonus is offered.

Qualifications

• Current Florida CNA license.
• CPR/BLS certification.
• Valid Florida driver’s license with acceptable driving record.
• High school diploma or GED preferred.
• Minimum of six months of relevant experience preferred; hospice or terminal care experience preferred.

Responsibilities

Position Summary

The Hospice Aide provides compassionate, hands-on personal care to terminally ill patients in various settings. The aide supports patient comfort, dignity, and safety while adhering to the established plan of care. The role involves direct care tasks, assisting with mobility, and supporting family caregivers, with flexibility to work weekends, holidays, and across different care sites as needed.

Essential Functions

• Provide personal care services in accordance with the plan of care and hospice regulations.

Qualifications

Knowledge and Critical Skills

• Understanding of personal care principles and hospice care standards.
• Ability to communicate effectively with patients, families, and team members.
• Demonstrated ability to work as part of an interdisciplinary team.
• Strong organizational and documentation skills

Experience

• Minimum of six months of relevant experience preferred.
• Experience in hospice or terminal care strongly preferred.

License / Certification

• Current Certified Nursing Assistant (CNA) license in the state of Florida.
• Current CPR/BLS certification.
• Valid Florida driver’s license with acceptable driving record.

Education

• High school diploma or GED equivalent preferred.

Other Requirements

• Required immunizations and screenings (e.g., TB, Hepatitis, Flu).
• Clearance for communicable diseases prior to patient contact.
• Reliable transportation and ability to shift work locations.
