Zusammenfassung

Heart to Heart Hospice is seeking a dedicated Hospice Aide (CNA) to support patient care under RN supervision. You will assist with transfers, mobility, and documentation, ensuring compassionate, compliant care.

The role emphasizes infection control, clear communication with families and staff, and timely record-keeping. A valid CNA license and at least one year of hospice or nursing assistant experience are required.

Qualifikationen

• Current CNA license per Nurse Aide Registry.
• High School Diploma or GED preferred.
• At least one year experience as Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice or long term care facility.
• Unencumbered certification in applicable registry.
• Must have current driver's license, reliable transportation, and provide ongoing auto liability insurance.

• Provide direct patient care to patient under direction of the RN and according to the Hospice Aide/Homemaker Plan of Care.
• Assist patient with safe transfers and ambulation per agency policy.
• Report and document pertinent information to ensure continuity of care.
• Practice infection control principles.
• Promote positive, respectful communication with patient/family and staff.
• Timely submission of all documentation.
• Perform other duties as required.
• Ensure compliance with local, state and federal laws, Medicare regulations, and policies.

What You Must Have:

• High School Diploma preferred or GED
• Must have an unencumbered certification according to the applicable Nurse Aide Registry
• Licensure: Current CNA license
• At least one year experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice or long term care facility
• Demonstrates interest in the welfare of ill and elderly; a heart for providing compassionate care
• High integrity, including maintenance of confidential information
• EMR experience a plus
• Must have current driver's license, reliable transportation, and provide ongoing valid and current auto liability insurance

What You Will Do:
