Position Summary

The Choice Hospice Aide is responsible for providing care to patients, including assistance with personal healthcare and related services in their residence. The aide helps ensure a safe and clean environment, working cooperatively with patients and families, and sharing observations with the supervisor. Our Hospice Aides are a vital part of the care team, helping us fulfill our hospice mission.

As a Hospice Aide, you will:

1. Provide direct patient care under the supervision of the RN and according to the Aid Plan of Care.
2. Assist patients with safe transfers and ambulation following agency policy.
3. Report changes and document care provided to ensure continuity of care.
4. Practice infection control principles to maintain a clean, safe, and comfortable environment.
5. Communicate positively, supportively, and respectfully with patients, families, and colleagues.
6. Respect patient privacy and property, fostering a respectful environment.
7. Assist with proper nutrition and fluid intake for patients.
8. Contribute to efficient agency operations and demonstrate effective time management.
9. Perform treatments and procedures as per agency policy.
10. Support the agency's philosophy and policies to ensure quality care.

Requirements:
