Job summary

Anchor Health is seeking an After-Hours Hospice LVN/RN to provide skilled, on-call nursing care outside of standard business hours. The role emphasizes independent decision-making, symptom management, and emotional support for patients and families after 5:00 PM.

The After-Hours Hospice LVN/RN works with the interdisciplinary team to support individualized care plans, communicates changes promptly, and ensures documentation is complete and timely.

Qualifications

• Current RN or LVN license in good standing.
• Hospice or home health experience preferred.
• Strong clinical judgment and ability to work independently.
• Excellent communication and problem-solving skills.
• Compassionate presence in emotionally sensitive situations.
• Valid driver’s license.
• Reliable, insured vehicle in good working condition (per state and organizational requirements).

Responsibilities

• Respond to after-hours patient calls and urgent care needs
• Perform skilled nursing assessments and symptom management
• Provide emotional support and education to patients and families
• Implement and update patient plans of care as needed
• Communicate patient condition changes to the interdisciplinary team
• Collaborate with physicians and caregivers regarding patient needs
• Complete accurate and timely clinical documentation
• Uphold hospice philosophy, policies, and regulatory standards

Position Overview

The After-Hours Hospice LVN/RN provides professional nursing care to hospice patients during evenings, nights, and weekends. This role ensures continuity of care, symptom management, and emotional support for patients and families after 5:00 PM.

The nurse works independently while maintaining close communication with the interdisciplinary team, physicians, and leadership to support individualized plans of care.

Schedule

• 5pm-8am
• Weekends and holidays as assigned
• On-call/after-hours coverage
