Anchor Health is seeking an After-Hours Hospice LVN/RN to provide skilled nursing care outside of standard hours, including evenings, nights and weekends, in Vallejo, California. The role offers independent practice with strong communication to the interdisciplinary team and leadership.

The position requires an active RN or LVN license, hospice/home health experience preferred, and a valid driver’s license with a reliable, insured vehicle.

Responsibilities

• Collaborate with physicians and caregivers regarding patient needs
• Complete accurate and timely clinical documentation

Anchor Health is seeking an After-Hours Hospice LVN/ RN to provide skilled, on-call nursing care outside of standard business hours.

Position Overview

The After-Hours Hospice LVN/RN provides professional nursing care to hospice patients during evenings, nights, and weekends. This role ensures continuity of care, symptom management, and emotional support for patients and families after 5:00 PM.

The nurse works independently while maintaining close communication with the interdisciplinary team, physicians, and leadership to support individualized plans of care.

Schedule

• 5pm-8am
• Weekends and holidays as assigned
• On-call/after-hours coverage

Key Responsibilities
