Job summary

Chapters Health System is seeking an Admission RN to assess patients at their place of residence or in hospitals, initiating care and ensuring a seamless transition to Care Navigation. You will work as a patient advocate, coordinating with physicians and care teams to manage admissions and discharges.

The role requires active RN licensure, at least one year of nursing experience (hospice or hospital preferred), EMR/EHR familiarity, and strong communication skills in English (Spanish a plus).

Qualifications

• Current RN license in state where working
• Minimum 1 year nursing experience; hospice or hospital preferred
• Experience with EMR/EHR systems
• Strong clinical assessment; ED/ICU background preferred
• Ability to communicate in English; Spanish a plus

Responsibilities

• Complete clinical admissions for all services, explain offerings empathetically
• Identify admission objections and provide responses
• Educate patient/family on admission requirements and needs
• Initiate Plan of Care with physician orders
• Coordinate with Care Navigation for timely follow-up
• Maintain SBAR communications and ensure efficient discharge planning
