Emmanuel Hospice seeks an Admissions Registered Nurse to welcome patients and families at the start of their hospice journey. The role covers admissions in homes, hospitals, or facilities across Grand Rapids and surrounding areas, with a sign-on bonus and comprehensive benefits.

The RN will perform initial clinical assessments, provide compassionate education, and collaborate with physicians on eligibility, ensuring seamless transition into hospice care and strong patient support.

Qualifications

• Current RN license in Michigan.
• Minimum of 2 years clinical experience in hospice, palliative care, or acute care.
• Exceptional interpersonal and communication skills.
• Ability to explain medical and hospice-related information with compassion and clarity.
• Strong organizational and documentation skills.
• Valid driver’s license and willingness to travel locally.

Responsibilities

• Be the first clinical contact for patients and families entering hospice.
• Conduct initial hospice information sessions and admissions in patients’ homes, hospitals, or facilities.
• Provide education, clinical assessments, and collaborate with physicians on eligibility.
• Explain hospice services, goals, and benefits to patients and families.
• Complete all required documentation and consent forms in a timely manner.
• Coordinate with the interdisciplinary team for a smooth transition into hospice services.
