Job summary

Amedisys Hospice of Brevard in Melbourne, FL is seeking an RN Admissions Nurse to join our compassionate team. This role coordinates admissions, conducts initial assessments of patients' needs, and helps establish the care plan with the attending physician and interdisciplinary team.

The ideal candidate holds an active RN license, 2+ years in hospice or home health, CPR certification, and a valid driver's license with reliable transportation.

Qualifications

• Current RN license in Florida.
• Minimum 2+ years of RN experience in hospice or home health within last 3 years.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Coordinates admissions of hospice patients by making skilled assessments of patients initial physical, psychosocial and spiritual needs.
• Coordinates the establishment of the initial plan of care with the attending physician, medical director and the interdisciplinary team.
• Provides skilled interventions and instructions to patient and caregivers to provide optimum symptom control and patient comfort under the direction of physician.
• Functions as a liaison between disciplines and facilities during the admission process.
• Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies.
