Haven Hospice in St. Augustine, FL is seeking a passionate Hospice Admissions RN to join our team.

You will visit patients in homes, hospitals, and facilities to discuss goals, obtain consents, and coordinate hospice start-up with the interdisciplinary team. Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year of hospice nursing experience required, 3+ years preferred.
• Must be a licensed driver with an automobile insured in accordance with state requirements.

Responsibilities

• Visit patients (homes, hospitals, nursing facilities) referred for hospice care.
• Explain hospice services, discuss goals and care with patients and families, obtain consents.
• Obtain medical history, current medications, and symptoms.
• Perform comprehensive clinical assessments to determine eligibility for hospice per Medicare/insurance guidelines.
• Coordinate the start of services (nursing, aides, equipment, medications).
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
