Job summary

Haven Hospice in Palatka, FL is seeking a Registered Nurse to join our admissions team. You will visit patients in homes or facilities, discuss goals of care, collect medical histories, and determine eligibility for hospice services based on Medicare guidelines.

This role offers flexible scheduling within a 9a-6p/10a-7p/11-8 framework, teamwork with an interdisciplinary group, and opportunities for professional growth as part of a supportive, patient-centered organization.

Qualifications

• Graduate of an accredited nursing program with active FL RN license.
• 1 year hospice nursing experience required; 3+ years in similar settings accepted.
• Must have a valid driver’s license and insured vehicle.

Responsibilities

• Visit patients referred to hospice in homes, hospitals, or facilities.
• Explain hospice services, discuss goals of care with patients and families, obtain consent and documents.
• Obtain medical history, current meds, and symptoms.
• Perform clinical assessments to determine hospice eligibility per Medicare/insurance guidelines.
• Coordinate start of services (nursing, aides, equipment, medications).
• Collaborate with interdisciplinary team and maintain timely documentation.
