Job summary

MissionHires seeks a compassionate Hospice Admissions RN in Columbus, OH to manage the admission journey from referral through care coordination. You will assess eligibility, educate families, obtain consent, and collaborate with physicians, referral sources, and the interdisciplinary team to ensure a smooth transition into hospice services.

This role requires strong clinical assessment, accurate documentation, and clear communication with patients and families during difficult times.

Qualifications

• Current RN licensure with Ohio State Board of Nursing.
• Valid Ohio driver's license and access to a vehicle.
• CPR and First Aid certification current.
• Hospice admissions or intake experience preferred.
• Knowledge of payer systems (Medicare, HMOs, managed care).
• Strong organizational skills and ability to multitask.
• Excellent assessment, listening, and customer-service skills.
• Ability to handle confidential information and admission records.
• Ability to work independently and in an interdisciplinary team.
• Flexible hours including weekdays, weekends, and evenings.
• Fluent English (read, write, speak).

Responsibilities

• Receive hospice referrals and coordinate admissions.
• Assess patients for hospice appropriateness and determine coverage needs.
• Conduct comprehensive nursing assessments and initiate care plans.
• Explain hospice services to patients and families; provide education during admission.
• Obtain consents, medical records, and admission documentation.
• Identify and address comfort-care needs while supporting spiritual and cultural needs.
• Document assessments, goals, and care interventions accurately.
• Coordinate services on admission and communicate with care team and external agencies.
• Share relevant information to support the interdisciplinary plan of care.
• Collaborate with the team to manage resources and costs.
• Build relationships with referral sources and provide professional service.
• Provide support to patients and families at the time of death.
• Participate in staff orientation and in-service training.
• Follow safety and infection-control policies; report hazards.
