Job summary

VNAcare in Murrieta, CA is seeking an Admissions RN to join our hospice team. The role performs patient admissions, coordinates care with physicians, hospitals, and referral sources, and ensures timely Hospice enrollment.

Requirements include CA RN license and at least one year of acute-care experience; hospice and EMR experience are preferred. This full-time daytime position offers a competitive salary, mileage reimbursement, and a comprehensive benefits package.

Qualifications

• Current CA RN license required.
• Minimum of 1 year acute-care experience.
• Hospice experience preferred; EMR experience preferred.

Responsibilities

• Maintains ongoing communication with physicians, hospital staff, and referral sources regarding patient care and available agency services.
• Evaluates potential Hospice patients and reports findings to the attending physician to obtain orders for Hospice care.
• Works with acute hospitals, SNFs, and medical groups to plan and coordinate Hospice services for referred patients and arrange supplies/equipment for discharge.
• Completes the Hospice admission process with required forms and documentation to insure continuity of care.
• May provide nursing care to Hospice patients within standards of practice and scope of practice.
