Job summary

Advocate Health Care is seeking a Registered Nurse (RN) to lead hospice admissions across Oak Brook, Lombard, Park Ridge, Elgin and surrounding areas. Shift options include day/PM, with a schedule Monday, Thursday, Friday and occasional Tuesday/Wednesday, plus one weekend per month and two holidays per year.

You will assess patients, coordinate with the interdisciplinary team, obtain prognosis and informed consent, review insurance coverage, and educate families about hospice services.

Qualifications

• Active RN license in the state of practice.
• BLS certification (may obtain within 6 months).
• Valid driver’s license.

Responsibilities

• Lead admission to hospice services for patients.
• Coordinate with the interdisciplinary team for each caseload.
• Obtain patient history and perform physical assessment.
• Explain hospice services and obtain informed consent.
• Review insurance/Medicare eligibility and assist billing.
• Assess readiness and develop the plan of care with family.
• Coordinate services and equipment with IDT.
• Provide hospice education to staff as directed.
