Job summary

VITAS Healthcare in Illinois is seeking a Hospice Admissions Nurse to serve as the compassionate bridge between hospital or community care and hospice services. You will guide patients and families through the admission process with clarity and empathy, ensuring a smooth transition to end‑of‑life care.

The role requires an active RN license and at least 2 years of nursing experience (hospice, palliative, ER, ICU, home health or case management preferred).
