Job summary

VITAS Hospice Admissions Nurse serves as the compassionate bridge between hospital or community care and the comfort of hospice, guiding patients and families through transitions with clarity, empathy, and confidence.

This role requires RN with valid state license and at least 2 years of nursing experience (hospice, palliative, ER, ICU, home health, or case management preferred). You’ll conduct comprehensive assessments, educate about hospice and Medicare benefits, and coordinate admissions.

Qualifications

• RN with a valid state license.
• Minimum 2 years of nursing experience; hospice, palliative, ER, ICU, home health or case management preferred.
• Strong clinical assessment and critical thinking skills.
• Compassionate communicator with excellent interpersonal skills.

Responsibilities

• Conduct comprehensive clinical assessments to determine hospice eligibility.
• Educate patients and families about hospice services and Medicare benefits.
• Collaborate with physicians, case managers, and interdisciplinary teams.
• Initiate and complete the admission process, including documentation and coordination.
