Job summary

VITAS Healthcare is seeking a dedicated Hospice Admissions Nurse to bridge hospital or community care with hospice services. You will guide patients and families through a pivotal transition with empathy, clarity, and clinical confidence.

As an Admissions Nurse, you will perform comprehensive assessments, educate families about Medicare benefits, coordinate with physicians and care teams, and manage the admission process from start to finish.

Qualifications

• Active RN license required.
• Minimum 2 years nursing experience (hospice, palliative, ER, ICU, home health, or case management preferred).
• Strong clinical assessment and critical thinking skills.
• Compassionate communicator with excellent interpersonal skills.

Responsibilities

• Conduct comprehensive clinical assessments to determine hospice eligibility.
• Educate patients and families about hospice services and Medicare benefits.
• Collaborate with physicians, case managers, and interdisciplinary teams.
• Provide emotional support and clinical guidance during the decision-making process.
• Initiate and complete the admission process, including documentation and coordination.
