Job summary

VITAS Hospice admissions nurse role bridges hospital or community care with hospice. You will conduct comprehensive clinical assessments to determine eligibility, educate patients and families about hospice services and Medicare benefits, and coordinate with physicians and interdisciplinary teams.

Qualifications

• Registered Nurse (RN) with a valid state license.
• Minimum of 2 years of nursing experience (hospice, palliative, ER, ICU, home health or case management preferred).
• Strong clinical assessment and critical thinking skills.
• Compassionate communicator with excellent interpersonal skills.
• Active listener who builds trust and rapport.
• Confident in guiding others through uncertainty, addressing concerns with empathy.

Responsibilities
