Job summary

Compassus is seeking a Hospice Admission Registered Nurse (RN) in Richmond, VA to assess hospice eligibility, explain services to patients and families, and coordinate with the Inter-Disciplinary Team to develop initial plans of care. You will address patients’ initial needs and support families during care transitions.

Requires an active RN license in Virginia, at least two years of RN experience (hospice experience preferred), a valid driver’s license, and CPR/BCLS certification to deliver

• Two years of experience as a registered nurse; hospice experience preferred.
• Current CPR and BCLS certification.
• Current driver’s license.
• RN license in the state you work.

Responsibilities

• Assess patients for hospice eligibility and explain hospice care services to patients, families/caregivers.
• Collaborate with the Inter-Disciplinary Team regarding initial plan of care and patient condition.
