Job summary

Compassus in Santa Fe, NM is seeking a Hospice Admissions Registered Nurse to assess patients for hospice eligibility and collaborate with the care team to address initial needs. The role involves explaining hospice services to patients and families and coordinating with the Inter-Disciplinary Team on the initial plan of care.

The RN must hold an active license in the state, have at least two years of RN experience (hospice preferred), possess a valid driver's license, and maintain CPR/BCLS

Responsibilities

• Enable patients to spend quality time with their loved ones and doing the activities they enjoy

• Ensure all patient’s initial needs are addressed
• Collaborate with the Inter-Disciplinary Team members regarding initial plan of care, patient’s condition, and other essential information
