Job summary

Cabrillo Hospice in California is seeking a compassionate RN Admissions Nurse to provide high-quality end-of-life care. As the first point of contact, you will conduct home assessments and guide families through the admission process.

The role offers a competitive pay of $62 to $67 per hour and emphasizes meaningful work within a supportive clinical team. Applicants must have a valid California RN license and a minimum of one year of hospice experience.

Qualifications

• Minimum 1 year of hospice experience required.
• Valid driver's license, reliable transportation, and auto insurance.

Responsibilities

• Conduct in-home hospice admission assessments and develop initial plans of care.
• Collaborate with physicians to confirm eligibility and coordinate admissions.
• Educate patients and families on hospice services and care expectations.
• Complete accurate and timely EMR documentation within 24 hours.
• Travel throughout Orange County and Inland Empire.
• Work closely with the interdisciplinary team to ensure continuity of care.

Location: Inland Empire
Schedule: Full-Time | Field-Based
Pay: $62–$67/hour

Role Overview

As an RN Admissions Nurse, you will be the first clinical point of contact for patients and families entering hospice services. You will conduct assessments, guide the admission process, and ensure a smooth transition into care.
