We are experiencing growth and are seeking compassionate RNs with a good heart to join our hospice team and family. This role is ideal for individuals who are great communicators and love customer service.

Responsibilities include:

• Completing initial and ongoing comprehensive assessments of patients and families to determine hospice needs.
• Providing professional nursing care.
• Initiating and revising the plan of care based on patient status and needs.
• Developing a care plan with goals based on nursing diagnosis, including palliative nursing actions, involving the patient and family in planning.
• Counseling patients and families to meet nursing and related needs.
• Meeting weekly with the interdisciplinary team (IDT).

Qualifications:

• New nurses are welcome to apply.
• Graduate of an accredited nursing school.
• Current licensure in the state and a valid driver’s license.
• Excellent observation, verbal and written communication, and problem-solving skills.
• One year of hospice experience preferred but not required.
