Job summary

Envision Hospice is seeking a compassionate RN Hospice Admission Nurse to perform admissions and patient assessments across King County. You will evaluate eligibility, review histories, coordinate with the care team, and educate patients and families.

Care is provided in private homes and facilities, with occasional travel to Snohomish, Kitsap, Thurston, and Pierce counties. This is a full-time role with comprehensive benefits and a competitive hourly wage.

Qualifications

• Active Washington State RN license and BLS certification.
• Valid driver’s license and reliable transportation.
• Minimum 1 year hospice or home health nursing experience preferred.
• Minimum 2-3 years healthcare-related experience required.
• Proficient with EMR systems and healthcare technologies.

Responsibilities

• Conduct patient assessments addressing physical, emotional, and spiritual needs.
• Collaborate with physicians and the interdisciplinary team on care plans.
• Educate and support patients and families during hospice admission.
• Complete required documentation accurately and promptly.
• Perform hospice admissions and, when not scheduled, provide routine visits including death visits.
• Stay current with hospice best practices, policies, and regulatory standards.

Schedule: Full-Time |Monday - Friday
Compensation: $52.00 - $63.00 per hour + Benefits

We are seeking a compassionate RN Hospice Admission Nurse to perform admissions and patient assessments, primarily serving patients throughout King County. You will evaluate patient eligibility, review medical histories, coordinate with the care team, and provide education and emotional support to patients and families. Care is provided in private homes, adult family homes, assisted living communities, and skilled nursing facilities. This position will be based in King County, with occasional travel to Snohomish, Kitsap, Thurston, and Pierce counties as needed.
