Job summary

Envision Hospice in King County, WA is seeking a compassionate RN Hospice Admission Nurse to perform admissions and patient assessments across private homes, adult family homes, and care facilities. You will evaluate eligibility, coordinate with the care team, and educate patients and families.

Responsibilities

• Conduct patient assessments addressing physical, emotional, and spiritual needs.
• Collaborate with physicians and the interdisciplinary team on care plans.
• Educate and support patients and families during hospice admission.
• Complete required documentation accurately and promptly.
• Perform hospice admissions and, when not scheduled, provide routine visits including death visits.
• Stay current with hospice best practices, policies, and regulatory standards.

Location: King County
Schedule: Full-Time |Monday - Friday
Compensation: $52.00 - $63.00 per hour + Benefits

We are seeking a compassionate RN Hospice Admission Nurse to perform admissions and patient assessments, primarily serving patients throughout King County. You will evaluate patient eligibility, review medical histories, coordinate with the care team, and provide education and emotional support to patients and families. Care is provided in private homes, adult family homes, assisted living communities, and skilled nursing facilities. This position will be based in King County, with occasional travel to Snohomish, Kitsap, Thurston, and Pierce counties as needed.
