Qualifications

• Active Nursing License in MO/KS

Responsibilities

• The Hospice Nurse provides service for the personal, physical, emotional and social needs of the patient-family unit. Care involves telephone contact, home visits, and/or facility visits for assessment, symptom management, teaching and coordination of care.
• On 7on/7off rotations, evenings and weekends, the nurse plans and directs services to meet patient and family needs within their homes and communities after normal business hours.
• Nurse evaluates caregiver needs and coordinates with the interdisciplinary team to ensure quality hospice care.

HIRING: Hospice Licensed Practical Nurse of Registered Nurse

Schedule:

Monday - Thursday 5PM-8AM

Friday 5PM-Monday 8AM (Continuous)

Territory: East Kansas City Metro territory covering approximately a 30-mile radius: Jackson County, and portions of Clay, Lafayette, Cass, Ray and Johnson counties; on-call clinicians may be dispatched anywhere within the service area.

The 7on/7off hospice nurse supports the patient needs of the agency in the evening during the weekdays and over the weekend. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities after normal business hours.
