Three Oaks Hospice is seeking a compassionate On-Call Registered Nurse to join our team serving patients in Atlanta, Carrollton, and Lithia Springs. The role involves providing skilled nursing care in patients' homes and coordinating with their families and the care team.

The RN must hold an active state RN license, CPR certification, and have at least two years of clinical RN experience (hospice/home health preferred). MS Office proficiency is preferred.

Qualifications

• Graduate of an accredited nursing program.
• Current RN license in the state.
• Two years’ RN experience in clinical care; hospice/home health preferred.
• Current CPR certification; strong computer skills.
• Proficient with Microsoft Office; strong communication skills.

Responsabilités

• Administer skilled nursing care for clients in their home.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency.
• Ensure care plans are implemented and updated; document care.
• Assist with care coordination during on-call shifts.

We are looking for a passionate On-Call Registered Nurse RN to join our leading team of hospice care professionals and make a difference, by listening, caring, and serving.

POSITION SUMMARY

Primary functions are to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency; and assumes the responsibility for coordination of care.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing
• Current state license as an RN and current state driver’s license
• Two years’ experience as an RN in a clinical care setting; hospice or home health experience preferred
• Current CPR certification
• Proficient skill using a computer is required; Microsoft office suite experience is preferred
• Strong interpersonal skills and ability to provide effective support to the patient and family
• Demonstrate skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles
