• Perform daily pre-admission, admission, and concurrent utilization reviews.
• Escalate utilization and system problems that are unresolved.
• Conduct rounds and review patient admissions under Kaiser Skilled Nursing Facility benefit.
• Utilize knowledge of Medicare regulations to determine medical necessity.
• Monitor the progression of the plan of care and facilitate discussions with multidisciplinary teams.
• Educate healthcare team members on utilization and cost containment initiatives.
• Collaborate with patients, families, physicians, and staff regarding care provisions.
• Develop, evaluate, and coordinate comprehensive discharge plans.
• Monitor care processes and assist in developing guidelines for patient care.
• Identify opportunities for cost-savings and improvements in quality of care.
