The Homeless Services Specialist will provide outreach services to Homeless individuals including youth, and families with substance use disorders (SUDs) or co-occurring mental health conditions and SUDs (COD). Staff will use Motivational Interviewing and SMART Recovery services, individual supportive care, and perform case management duties. Homeless Services Specialist will conduct outreach, enroll adults in treatment, case management, and recovery support (including benefit assessment by SOAR certified staff) and will proactively link youth to youth-specific services.
Homeless Services Specialist will focus on 1) behavioral health outreach, treatment, and recovery-oriented services; 2) coordination of housing and services to support the implementation and/or enhance long-term sustainability of integrated community systems that provide permanent housing and supportive services; and 3) effort to engage and connect individuals with SUD or COD, who are experiencing homelessness to resources for health insurance, Medicaid, and mainstream benefits programs (e.g. Supplemental Security Income(SSI)/Social Security Disability Insurance (SSDI), Temporary Assistance for needy Families (TANF), Supplemental Nutrition Assistance Program (SNAP), etc.).
