Job summary

Rockford seeks a Home Care Supervisor to oversee services delivered to individuals in their homes and communities, ensuring compassionate, compliant care and adherence to agency policies.

You will supervise frontline staff, coordinate schedules, and monitor documentation and EVV to maintain quality service and branch reporting.

Qualifications

• High school graduate or equivalent.
• Valid driver's license.
• Adequate vehicle insurance coverage.
• 1–3 years in home care/healthcare/case management preferred.
• 1–2 years supervising preferred.

Responsibilities

• Oversee home care services provided to individuals in their own homes and communities.
• Ensure employees deliver services with care and respect, in line with agency policies and industry standards.
• Reports to Branch Manager/Lead Supervisor.

• Oversee services provided to individuals in their own homes and communities, who need assistance caring for themselves as a result of old age, sickness, disability, and/or other conditions. Home care may include assistance with the activities of daily living, housecleaning, laundry, meal preparation, transportation, companionship, and respite.
• Home Care Supervisors ensure that employees deliver services in a caring and respectful manner, in accordance with relevant Agency policies and industry standards.

Qualifications/Education

• High school graduate or equivalent.
• Current driver’s license.
• Proper Vehicle Insurance Coverage
• 1-3 years of experience in home care, healthcare, or case management preferred
• 1-2 years' experience in supervising preferred

• IDoA regulations and provider requirements
• Electronic Visit Verification (EVV)
• Documentation standards
