Qualifications

Join the Cleveland Clinic team, where you will work alongside passionate caregivers and provide patient-first healthcare. A caregiver in this role will serve patients in the following counties: Erie, Huron, Lorain, Medina, Cuyahoga and others as needed

Cleveland Clinic’s Home Care team helps patients continue their treatment and learn/relearn any skills necessary to stay safe and independent again. As an RN, you are responsible for providing direct nursing care, participating in a team-based care model, implementing and monitoring patient care plans, and reporting patients' needs after every visit.

A caregiver in this position works days from 8:00AM – 5:00PM, with on-call requirements.

A caregiver who excels in this role will:

  • Provide direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization.
  • Participate in a team-based care model led by the Care Coordinator in each region.
  • Implement and monitor patient care plans established by, and report patient needs after each visit to, the Care Coordinator for further case management.
  • Perform accurate and complete starts of care, resumptions, recertification's and routine visits as requested and within specified time frames.
  • Perform medication reconciliation at each visit.
  • Assess patients for safety risks, abuse and neglect.
  • Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions.
  • Report abnormal findings and/or changes in patient condition to the Physician and update Care Coordinators with those changes and any follow-up needed.
  • Communicate any patient's fall out/missed visits to the Care Coordinator immediately.
  • Participate in and support quality improvement projects and participate in department committees as assigned.

Minimum qualifications for the ideal future caregiver include:

  • Graduate from an accredited School of Professional Nursing
  • Current state licensure as a Registered Nurse (RN)
  • One year of current clinical experience as an RN
  • One year of experience in an acute medical/surgical or homecare setting
  • Valid Ohio driver’s license and proof of $100,000/$300,000 automobile insurance coverage

Preferred qualifications for the ideal future caregiver include:

Physical Requirements:

  • Full range of motion, manual and finger dexterity and eye-hand coordination
  • Standing and walking for extensive periods of time
  • Corrected hearing and vision to normal range
  • Some exposure to communicable disease or bodily fluids
  • Medium Work, exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects
  • Physical Demand requirements in excess of those for Light Work
  • Must be ambulatory
  • Ability to drive to patients' homes, able to climb stairs, pull, push, and facilitate movement of the patient
  • Ability to read, write and communicate with patients

Personal Protective Equipment:

  • Follow standard precautions using personal protective equipment as required
