Job summary

Samaritan Health is seeking a registered nurse to provide in-home care within the Samaritan Home Health program. The RN will plan, coordinate and document patient care in patients' homes, ensuring safety and quality aligned with NYS licensure and our standards.

Required: NYS RN license; previous home health experience preferred; BLS; weekend and holiday on-call availability. ACLS preferred. Valid NY driver’s license and auto insurance proof are needed.

Location: Samaritan Home Health Department: 08.6994 SHH HOME HEALTH

Pay Range: $36.54 - $60.29

Graduate of an accredited school of nursing, Current NYS RN License required. Previous RN experience is preferred. New Graduate RN's welcome to apply. Previous Home Health Care experience preferred. Minimum 1 year of medical-surgical experience.

Primary Responsibilities:
  • Planning, coordinating, providing and documenting the care of patients in their place of residence.
  • Clinical practitioner who coordinates and implements patient care specific to Jefferson County.
  • Ensures that compassionate, engaged care meets or exceeds quality standards.
  • Care is provided in an efficient and safe manner consistent with the NYS licensure and Samaritan Home Health standard of care.
  • Utilizes nursing skills to determine the biophysical, psychosocial, environmental, self-care, cultural, ethnic and spiritual and educational needs of the patient/family/significant other to develop, implement and revise the plan of care.
  • In doing so, performs accurate assessments, interprets and correlates finding, establishes plan of care and implements short and long-term goals.
  • The nurse observes patient progress, records pertinent observations, implements physician and nursing treatment plans and evaluates outcomes.
  • The RN recognizes their responsibilities as specified by law and abides within those responsibilities.
  • Provides direct supervision to LPNs and HHAs.
  • Provides on-call services as required.
  • BLS required.
  • ACLS preferred.
  • Valid NYS Driver's license and proof of auto insurance.
  • Must be available weekends and Holidays and provide on-call services as required.
