Homecare Physical Therapist | New Jersey

Salary Range: $90,000 – $120,000 per year

Location: Passaic, New Jersey

Job Type: Full-Time

Schedule: Monday to Friday, 9 AM – 5 PM

We are seeking a skilled and compassionate Physical Therapist to join our growing homecare team. The ideal candidate is self-motivated, organized, and committed to providing exceptional care in a home setting. You’ll work one-on-one with patients to develop and implement individualized treatment plans tailored to their specific needs.

We are dedicated to delivering high-quality, compassionate care directly to patients in their homes. Our team of licensed professionals works closely with patients, families, and physicians to support recovery, improve mobility, and enhance quality of life in the comfort of home.

Key Responsibilities:

• Perform in-home physical therapy evaluations and develop individualized care plans.
• Provide direct physical therapy interventions including exercise, manual therapy, and patient/caregiver education.
• Monitor patient progress, adjust treatment plans as needed, and document all visits accurately in accordance with regulatory standards.
• Coordinate care with physicians, nurses, and other interdisciplinary team members.
• Ensure a safe, positive, and effective therapy environment in patients’ homes.

Additional Responsibilities:

• Evaluate and develop personalized treatment plans for patients with orthopedic and musculoskeletal conditions.
• Administer manual therapy techniques, therapeutic exercises, and modalities.
• Educate patients and their families about treatment plans, exercises, and progress.
• Collaborate with referring physicians, case managers, and other healthcare providers.
• Document patient care in compliance with state and federal regulations.
• Contribute to a positive clinical environment and support team goals.

Minimum Qualifications:

• Doctorate or Master’s Degree in Physical Therapy from an accredited program.
• Current New Jersey Physical Therapist License.
• Minimum of 1 year of clinical experience (home health experience preferred but not required).
• Reliable transportation and a valid driver’s license.
• Strong communication and time management skills.
• Comfortable working independently in patients' homes.
