Job summary

First Day Homecare is seeking a licensed Practical Nurse (LPN) to provide skilled pediatric home nursing services under physician orders. This role involves 1:1 care for infants, with flexible scheduling and the possibility of night and day shifts.

You will administer medications, monitor patients, and document orders in the medical record while coordinating with the care team to ensure adherence to the patient’s care plan. A valid LPN license and CPR certification are required.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current license to practice as an LPN in the state of operation.
• Current CPR certification.
• Demonstrated written, verbal, and interpersonal communication skills.
• Licensed driver with automobile insured in accordance with state and/or Agency requirements.

Responsibilities

• Provides professional nursing care as defined in the state Nurse Practice Act.
• Provides care in accordance with medical practitioner orders and the client’s care plan.
• Maintains orders for clients and documents changes in orders in the medical record following Agency procedures.
• The scope of clinical practice includes personal care services, monitoring and reporting findings, medication administration, client teaching, emergency intervention, and specialized pediatric services such as tracheostomy care, ventilator management, and tube feedings.
• Works collaboratively with other members of the client’s care team.
• Promotes personal safety and a safe environment for clients and coworkers.
• Performs job in compliance with Agency policies, procedures, and applicable payor and accreditation standards.
• Participates in the coordination of care and attends Agency meetings as requested.
