MaineGeneral Health is seeking an experienced Registered Nurse (RN) to join the HomeCare North Team as a Case Manager in Waterville, ME. The role offers 32 or 40 hours per week with call, weekend and holiday rotation within the North Team’s territory.

The ideal candidate holds a Maine RN license with at least 1 year of acute/med-surg experience; ED/ICU/home care experience is a plus. A valid driver’s license and EMR proficiency are required.

Qualifications

  • Active Maine RN license or ability to obtain in a timely fashion.
  • At least 1 year of acute or med/surg nursing experience.
  • Previous ED, ICU, CCU, or home care experience preferred.
  • Valid driver’s license and acceptable driving record.
  • Strong technology/computer skills including EMR experience.

Responsibilities

  • Consults with nursing staff and multidisciplinary team to evaluate patient status and care appropriateness, including admission, length of stay, transfer and discharge.
  • Provides direct treatment and management of health conditions and makes referrals for additional services.
  • Plans, implements, and evaluates patient care plans based on assessment to optimize outcomes and resources.
  • Participates in interdisciplinary team planning and provides discharge education.
  • Supports weekly scheduling of patient visits and collaborates with team.

We’re currently seeking an experienced Registered Nurse (RN) to join our HomeCare North Team as a Case Manager. This position is 32 or 40 hours per week (candidate choice). Requires call, weekend, and holiday rotation with team members. This position is based out of Waterville, but staff must be flexible to work in/travel to areas surrounding the North Team’s territory (Skowhegan & Waterville).

Position Highlights:
  • $15,000 Sign-On Bonus potential for nurses with at least 1 year of RN experience (New MGH hires only. Rehires within 1 year are not eligible. Bonus is paid out over 3 years as long as the employee stays in the job for at least 30 hours per week.)
  • Case Managers have flexibility and autonomy when scheduling patient visits.
  • We value our nurses’ opinions and encourage the sharing of ideas through many shared-governance committees.
  • We are deeply committed to providing ongoing education and support clinical certification and professional growth.
The ideal candidate will have:

Plans, implements, and evaluates patient care plans based on patient assessment to optimize outcomes and maximize available resources. Monitors, records, and communicates patient condition as appropriate.

Participates in interdisciplinary team planning to collaborate with other disciplines to determine appropriateness of care and other services needed. Provides discharge education to patients.
