Job summary

East Liberty Family Health Care Center seeks a Homebound Outreach and Quality Nurse to deliver in-home nursing and telephonic case management for patients with chronic conditions. Bilingual English/Spanish encouraged; vaccines required.

Full-time position in the East Liberty/Lincoln-Lemington service area with EMR documentation and CMS care management experience valued. Responsibilities include care planning, patient visits, chart reviews, and quality monitoring to meet HRSA and health plan

Qualifications

• RN degree and current state nursing licensure.
• Minimum 3 years of home care experience required.
• Geriatric nursing and MD office experience preferred; behavioral health a plus.
• Valid driver's license, insurance and ability to visit patient homes.
• Knowledge of EMR documentation and CMS chronic care management helpful.

Responsibilities

• Screen referrals to Homebound program and assess patient need.
• Update demographics, consents, sliding fee scale and insurance.
• Develop care plans and goals; follow chronic care module.
• Visit patients at home, record vitals, medications and labs.
• Triage calls and respond per protocol; document end-of-day notes.
• Monitor HRSA UDS and quality measures; ensure timely chart reviews.
• Prepare reports for QA/Leadership meetings.
