Job summary

A non-profit community health service in Detroit is seeking a Home-based Clinician to provide individual and family therapy to children with emotional disturbances. Job description

As a Certified Community Behavioral Health Clinic, CNS Healthcare's mission is to serve the people of our communities. Home-based Clinicians assist children with serious emotional disturbance and their families, who require access to an array of mental health services. Home-based Clinicians utilize a strength-based approach, emphasizing assertive intervention, parent and professional teamwork, and community involvement with other service providers. Clinicians provide in-home family and individual therapy, behavioral training and intervention, and case management services.

Our ideal candidate provides thorough and complete assessment, at intake and annually, to assess the client’s history and identify treatment needs. They provide crisis intervention when needed. Completion of risk assessments as indicated (e.g., suicide/homicide/potential for violence/substance use) with dispositions and recommendations that are included in IPOS. Our Home-base Clinician is responsible for monitoring, evaluating, and recording client progress with respect to treatment goals. Modify person-centered plans according to changes in client status.

Please note: As a community-based role, this position will render at least 80% of services in the beneficiary's home or other agreed-upon community-based location.

Compensation Range:

The starting pay for this position is $57,600 - $74,000 for limited licensed candidates, and $61,850 - $80,150 annual salary for fully licensed candidates, and is based on non-discriminatory factors such as skills and experience.

Essential Functions
  • Provide individual therapy, family therapy, group therapy, case management services in alignment with the individual plan of service (IPOS) utilizing evidence-based practices and a trauma-informed approach.
  • Perform comprehensive assessments, diagnosis, and evaluation of persons served as clinically appropriate.
  • Complete the appropriate screening and other assessments as clinically appropriate.
  • Focuses on improving therapeutic supports and functional skills in the areas of education, employment, social supports, wellness, self-management, and prevent out-of-home placement.
  • Provide crisis intervention when needed. Completion of lethality risk assessments as indicated (e.g. suicide/homicide/potential for violence/substance use) with dispositions and recommendations that are included in IPOS.
  • May collaborate with the case holder to support person-centered planning with assigned person served to identify needs and interventions.
  • Support the coordination of care for assigned cases by collaborating with internal and external providers.
  • Responsible for managing assigned caseload including ensuring that all individual documentation and authorizations are up-to-date, and following admission, transition, and discharge procedures.
  • Perform or facilitate the intake of new persons served as assigned.
  • Participate in team meetings, agency focus groups, work groups, and committees as assigned.
  • Support the development of agency staff and community partners by providing training as assigned.
  • May participate in internal and external presentations to provide education about home-based services.
  • May provide transportation for persons served.
  • Meet direct service productivity and documentation standards per agency and program expectations.
  • Maintain compliance with quality care initiatives including Meaningful Use (MU), Medicare Merit Based Incentive Payment System (MIPS), Commission on Accreditation of Rehabilitation Facilities (CARF), and Certified Community Behavioral Health Clinic (CCBHC) requirements.
  • Ensure that the CNS Healthcare standards of service are applied to interactions with individuals served, guests, and staff.
  • Assume responsibility for professional growth by current reading and attendance at educational programs and conferences.
  • Perform other related duties as assigned.
Qualifications

Required

  • Master’s Degree in Social Work, Counseling, Psychology, or Marriage and Family Therapy, AND
  • State licensure: Limited-Licensed Master’s Social Worker (LLMSW), Licensed Master’s Social Worker (LMSW), Limited-Licensed Professional Counselor (LLPC), Licensed Professional Counselor (LPC), Limited-Licensed Marriage and Family Therapist (LLMFT), Licensed Marriage and Family Therapist (LMFT), Temporary Limited-License Psychologist (TLLP), Limited-Licensed Psychologist (LLP), OR Licensed Psychologist (LP)
  • Child Mental Health Professional:
    • Possess or obtain Child Mental Health Professional (CMHP) designation within 90 days of hire, and then maintain throughout employment.
    • At least 1 year of experience in the examination, evaluation, and treatment of minors and their families.
  • Proficient with computers, Microsoft office products (including Outlook, Word, and Excel), and electronic health records (EHRs)

  • Ability to work closely with persons with mental or physical limitations

  • Valid and unrestricted Michigan Driver’s License
  • Reliable transportation and current proof of auto insurance

Preferred

  • Intellectual and Developmental Disability Professional:
    • Possess or obtain Qualified Intellectual Disability Professional (QIDP) designation within 90 days of hire, and then maintain throughout employment.
    • At least 1 year of experience involving the treatment of individuals with intellectual or developmental disabilities, can include internship or field placement experience.
  • Prior experience working with all age groups including children; solution focused brief therapy; substance use and co-occurring disorders treatment; stages of change and/or motivational interviewing.
  • Experience in the community mental health setting.
  • 1 year of clinical experience in individual and group psychotherapy under clinical supervision.
  • 1 year of master level experience in the examination, evaluation, and evaluation of children with severe emotional disturbances.
