Job summary

Bloom Healthcare is expanding its in-house wound care program to serve home-bound patients in Colorado, including assisted living communities and private homes. We seek a Nurse Practitioner or Physician Assistant to conduct comprehensive home visits, develop evidence-based treatment plans, and drive outcomes while collaborating with MDs, triage, IT, and scheduling teams.

Qualifications

• Master’s degree required.
• Colorado licensure required.
• Wound certification preferred.

Responsibilities

• Deliver wound care at home visits to 7-8 patients per day.
• Deploy evidenced-based wound care interventions.
• Determine appropriate follow-up frequency and manage ongoing wound care caseload.
• Demonstrate exceptional communication and relationship-building with care team members.
• Engage with population health data to drive patient outcomes.
• Attend ongoing educational opportunities.
• Commute to assigned assisted living communities and patients’ homes to provide care.
