Position Overview

Responsibilities

• Evaluating the effectiveness of the care plans for clients
• Assessing clients’ health, welfare, and unmet needs
• Developing backup plans and goals with clients
• Communicating with Care Coordinators to ensure clients receive appropriate and effective services for maximum independence
• Improving the well-being and healthcare outcomes for disabled children, adults, and seniors participating in at-home, long-term care programs
