Job summary

The Florida Department of Health in Pinellas County is seeking a HUMAN SERVICES COUNSELOR I to provide in-home family support, education, and case management for up to five years under Healthy Families America standards. You will coordinate with nurses, therapists, and other specialists to link families to community resources and monitor progress.

Responsibilities include home visits, screenings, documenting in HMS, maintaining confidentiality, and reporting to supervisors.

Qualifications

• At least one year of experience working with at risk children and/or families.
• Valid driver's license with daily access to a reliable personal vehicle.
• Ability to work a flexible schedule to include evenings and weekends, as needed.
• High school diploma or its equivalent, as mandated by the policies of Healthy Families Florida.
• Preferred - bilingual English/Spanish.

Responsibilities

• Establishes trusting relationships with families through home visits and case management.
• Provides parent education and coordination with health and social services.
• Maintains electronic health records and case notes.
• Meets grant goals and program requirements.
• Participates in supervision and staff meetings.
• Performs mandated reporter duties.
