Qualifications

• Master's degree in Speech-Language Pathology and ASHA CCC-SLP.
• New York State license to practice as a Speech-Language Pathologist.
• One year of supervised institutional experience and home care preferred.

Responsibilities

• Assesses patient’s speech and language abilities.
• Prepares initial assessments and periodic reassessments.
• Develops a plan of care aligned with the physician’s plan of care.
• Documents treatment outcomes in progress notes.
• Provides rehabilitative services for speech-language disorders.
• Instructs caregivers in home therapy methods.
• Participates in interdisciplinary team meetings.
• Ensures compliance with agency policies.
