Job summary

AltaMed Health Services in Anaheim, CA is seeking a Home Visit Program Coordinator to deliver medical services for complex home-bound clients, with a primary focus on Medicare Annual Wellness Visits and care coordination.

The ideal candidate has a High School diploma with Medical Assistant certification, at least two years of back-office and managed health care experience, and bilingual Spanish/English; a current BLS certification is preferred.

Qualifications

• High School graduate or equivalent with a Medical Assistant certification or equivalent required.
• Minimum of 2 years of back-office experience preferred.
• Minimum of 2 years of experience in a managed health care environment, preferably IPA, HMO, or health plan preferred.
• Experience in hospice, palliative care, or high-risk care settings preferred.
• A minimum requirement of a valid BLS certification or higher, following AHA/Red Cross guidelines.

Responsibilities

• Provide medical services under the Home Visit Program, focusing on Medicare Annual Wellness Visits and care coordination for home-bound clients.
• Coordinate care across the Home Visit Program to ensure appropriate and professional medical services.

Job Overview

The Coordinator, Home Visit Program provides medical services to the Home Visit Program for complex client care cases, ensuring appropriate and professional medical care, focusing primarily on Medicare Annual Wellness Visits, as well as providing care and coordination for home-bound complex care clients.

Minimum Requirements

• High School graduate or equivalent with a Medical Assistant certification or equivalent required.
• Minimum of (2) years of back-office experience preferred.
• Minimum of (2) years of experience in a managed health care environment, preferably IPA, HMO, or health plan preferred.
• Experience in hospice, palliative care, or high-risk care settings preferred.

Compensation

$26.91 - $33.53 hourly
