Job summary

Clarvida in Augusta, Georgia is seeking a Case Manager to work on-site in a community-based setting. You will engage clients to create and implement an individual service plan and help identify goals, needs, and supports.

Responsibilities include linking clients to community services, providing living-skills coaching (grocery shopping, budgeting, home upkeep, meal planning, social skills, coping strategies), and performing regular home visits to monitor progress and progress toward goals;

Qualifications

• Bachelor’s Degree in Social Work, Psychology or related field
• Experience in mental and behavioral health field
• Strong communication (verbal & written) and cultural sensitivity
• Valid Georgia driver’s license
• Criminal background check clearance

Responsibilities

• Engage client to develop and implement an individual service plan
• Link client to community services and supports
• Provide living-skills training (grocery shopping, budgeting, home upkeep, meal planning, social skills, coping strategies)
• Conduct regular home visits to monitor progress and progress toward goals
• Provide transportation as needed
