Responsibilities

• Perform 6-8 preventive visits daily to optimize chronic conditions.
• Perform timely new care visits and follow-up care as needed.
• Assume responsibilities as interim provider if there's no existing PCP.
• Co-manage patients with the PCP and serve as an extension of care.
• Coordinate with other physicians to smooth transitions and prevent readmissions.
• Offer medical direction to community-based organizations.
